Special Considerations for Working with Perinatal OCD Presented to Talkiatry

Course Description

For some individuals, symptoms of obsessive-compulsive disorder (OCD) develop or worsen during the perinatal period (pregnancy and/or postpartum) - this is commonly referred to as ‘Perinatal OCD.’  Perinatal OCD can be highly debilitating, difficult to recognize, and may go untreated.  The presenters will begin the course by describing the perinatal OCD presentation and discussing related research findings. Evidence-based therapeutic and pharmacological strategies for the treatment of perinatal OCD will be discussed.  Included case studies highlight several special treatment considerations to keep in mind when working with this population, including the importance of empathy and psychoeducation, the use of exposure and response prevention (ERP) during the perinatal period, incorporating peer and group support into the treatment plan, discussing shame and guilt with patients, the concept of paternal OCD, and the role of family accommodation.

Patrick B. McGrath, PhD

Dr. Patrick B. McGrath, PhD, is the Chief Clinical Officer for NOCD and a licensed clinical psychologist. Dr. McGrath specializes in treatment for anxiety, OCD, hoarding, school refusal, and trauma. He has given over 900 presentations on these topics at local, national, and international conferences, as well as at universities, hospitals, and private practices. Dr. McGrath is also an experienced continuing education instructor.  

Dr. McGrath earned his doctoral degree from Northern Illinois University and completed his two-year postdoctoral fellowship at the St. Louis Behavioral Medicine Institute’s Anxiety Disorders Center.  

Dr. McGrath previously opened and led the Ascension Illinois/Alexian Brothers OCD and Related Anxiety Disorders Intensive Outpatient, Partial Hospital, and Residential Treatment Programs. He has extensive clinical experience working with and providing supervision for complex OCD cases. He currently provides weekly consultation at NOCD for OCD, hoarding, and other anxiety-related cases to a national network of clinicians. 

Dr. McGrath is a member of the Scientific and Clinical Advisory Boards of the International OCD Foundation. He is a Fellow of the Association for Behavioral and Cognitive Therapies, and the Immediate Past President of OCD Midwest. Dr. McGrath also provided his expertise in OCD and anxiety disorders as a therapist on the Discovery Health Channel's documentary Anxious.

Jamie Fuesner, MD

Jamie Feusner, MD, Diplomate, American Board of Psychiatry and Neurology, obtained his medical degree and completed his psychiatry residency  at the University of California Los Angeles. He then completed a one-year clinical psychopharmacology fellowship followed by a two-year National Institute of Mental Health research fellowship.

Dr. Feusner's research program seeks to understand perceptual and emotional processing phenotypes across conditions involving body image and obsessions/compulsions, including body dysmorphic disorder, eating disorders, and OCD.  He has also investigated predictors of acute and long-term treatment response to cognitive-behavioral therapy. 

Dr. Feusner has extensive experience with clinical teaching of cognitive-behavioral therapy and pharmacotherapy. 

Dr. Feusner is the Chief Medical Officer for NOCD, is a Professor in the University of Toronto’s Department of Psychiatry, Division of Neurosciences & Clinical Translation, is a Clinician Scientist at the Centre for Addiction and Mental Health, is an Adjunct Professor at the Karolinska Institute, is a member of the board of directors of the International College of Obsessive Compulsive Spectrum Disorders, is on the editorial board of the Journal of Obsessive-Compulsive and Related Disorders, and is a member of the IOCDF Scientific and Clinical Advisory Board. He is the former Director of the UCLA Adult OCD Intensive Treatment program.

Mia Nuñez, PhD

Dr. Mia Nuñez, PhD is the Director of Clinical Integrity for NOCD and a licensed clinical psychologist. Dr. Nuñez oversees the NOCD network of clinicians and ensures clients receive high-quality psychological services. Dr. Nuñez has extensive experience with OCD, anxiety disorders, and eating disorders. 

Dr. Nuñez received specialized training in cognitive-behavioral therapy from Northwestern University's Anxiety Disorders Treatment Clinic. She was also a member of Northwestern University's Anxiety Laboratory and the Anxiety and Panic Laboratory at the Family Institute at Northwestern University. She continued to build her expertise in the cognitive-behavioral treatment of mood and anxiety disorders during her predoctoral psychology internship at the APPIC-accredited Rogers Behavioral Health Hospital.

Dr. Nuñez has worked as a supervising psychologist for several inpatient and residential specialty programs, including a child and adolescent service line for eating disorders. As a supervising psychologist, Dr. Nuñez supervised therapists in their delivery of exposure therapy for treatment of eating disorders. She also personally provided exposure therapy for complex cases. Dr. Nuñez has been an instructor for many courses, including exposure therapy for the treatment of eating disorders. She has published peer-reviewed journal articles and presented at conferences on the topic. Dr. Nuñez is a published author and contributor to research.
